Coach Registration
Thank you for being a coach this season!
Foothills Hockey Association is committed to bringing a rewarding hockey experience to each and every youth hockey player in the association and we rely on our coaches to help bring our mission to life.
All coaches are required to complete this registration form by providing their credentials and acknowledgement of association and SafeSport codes of conduct.

Prior to participating in any team activities:
- Register as a Coach with USA Hockey
- Complete National Background Screen
- Review USA Hockey and FHA SafeSport policies and procedures (includes policies on electronic communication, locker rooms, coaches behavior, travel, etc.)
- Complete USA Hockey Safe Sport Training. (Required on an annual basis.)
- Complete USA Hockey Age-Level Module for the level you are intending to coach.
- Complete HEADS UP Youth Sports Concussion Training. (One-time only training.)
- Complete FHA Coach Registration and receive acceptance from the FHA Hockey Director.
Prior to December 31:
- Complete Coaching Education Program Certification Clinic Requirements (as required by USA Hockey
2026-2027 Season
COACH REQUIREMENTS
Prior to starting this registration, all coaches must have a USA Hockey registration number for the current season.
Coaches and Volunteers may NOT have any contact with athletes or handle any association funds until both the background screen and SafeSport training are complete and you are cleared by USA Hockey.
Background Screen:
All volunteers must have the USA Hockey-mandated national background screening through NCSI on an annual basis. There is a $30 charge for this service and it takes approximately 10 business days to complete.
SafeSport:
All Volunteers MUST complete USA Hockey SafeSport Training (Core Training or Refresher Course) on an annual basis regardless of when the training was last completed.
